PURE REPLENISH PLANS

A six (6) month or twelve (12) month commitment is required for all memberships. If a membership is canceled prior to completion of the selected term, a $50 cancellation fee will be charged to the credit card on file. Each membership must have a valid credit card on file, which will be charged once monthly throughout the duration of the membership. If a monthly charge fails to process, an additional 5% processing fee will be added, and IV services will not be redeemable until payment is received. If the monthly charge does not successfully process on the 1st of the month, payment must be received before the monthly IV can be scheduled or administered. The patient is responsible for scheduling their monthly IV service. If a monthly IV is not used within that month, it will be forfeited. In the event of an unforeseen circumstance, the client must contact Dorica Brown or Barre Brown to discuss possible arrangements. If additional IV services are desired within the same month, the membership price will be honored for all additional IVs, including applicable free add-ons. IV services may be gifted to a friend or family member during the membership term with prior notice to Dorica Brown or Barre Brown, including the recipient’s name and preferred month.
